This Merchandise Order Form will allow you to order items from the Gift Shop that can be mailed to you.  Download this form to your computer in order to use the interactive features of this form.  After completing this form, print it and mail it to the Killingly Historical Society along with a check made payable to KHGS or to the Killingly Historical Society.

The Merchandise Order Form with Submit option will allow you to order items from the Gift Shop that can be mailed to you, and the Submit button will work with your EMAIL software to format an email to the KHGS Director with the order form attached to the email.   Download this form to your computer in order to use the interactive features of this form.  Note: this method assumes that you want to use a credit card or ACH method to purchase the items.  Do NOT include your bank or credit card information on the order form or in the email.  The KHGS Director will forward your email and order form to the KHGS Treasurer, who will send you an  invoice where you can securely input your bank or credit card information.
